There is an image of this cultivar on the Hosta Library. From the photo on the HL website, it appears to be a small size hosta with dark green, heart shape leaves and white medial (center) variegation. The pattern on the variegation shows white tissue between the green colored veins.  

The Hosta Library speculates that this plant is infected with a virus. So does The Hostapedia by Mark Zilis (2009).
